The Oxfordshire assistant coroner, Andrew Walker, who presided over the first inquiry into the deaths of eight British servicemen at the hands of US forces in a "friendly fire" incident, has the refusal of the Americans to cooperate with the inquest at the start of a second inquest. The Americans have refused to make those involved available or event to provide names or statements.
